LeafLine Labs operates in Eagan, St. Cloud, Hibbing, and St. Paul.]

Regulatory Assessment - The annual regulatory assessment is a nonrefundable, state-level fee which offsets LARA’s annual
operational costs. The regulatory assessment must be submitted before a license can be issued. The requirements in this subdivision do not apply after June 30, 2018.

Residency Requirements:
Under MCL § 333.27402(2)(g), if the applicant is an individual, the individual must have been a resident of Michigan for not less than a continuous 2-year period immediately preceding the date of filing the application.

Medical Marijuana Qualifying Patient Conditions:
1) Cancer associated with severe/chronic pain, nausea or severe vomiting, or cachexia or severe wasting.
2) Glaucoma.
3) HIV/AIDS.
4) Tourette Syndrome.
5) Amyotrophic Lateral Sclerosis (ALS).
6) Seizures, including those characteristic of Epilepsy.
7) Severe and persistent muscle spasms, including those characteristic of Multiple Sclerosis.
8) Inflammatory bowel disease, including Crohn’s disease.
9) Terminal illness, with a probable life expectancy of less than one year.*
* To qualify for the program, you must suffer from cancer or a terminal illness with a probable life expectancy of under one year, if your illness or its treatment produces one or more of the following: severe or chronic pain; nausea or severe vomiting; or Cachexia or severe wasting.
